Open up a USB hub (fins a 'C' socket version).
Find a miniature one, take the case off.
Snip the USB connector for and solder to the USB points.
That should keep it slim.
Only really useful for adding a mouse, keyboard, very slow USB pen drive, maybe a sound dongle.

Again it'll only be USB2 speeds (not USB3) if you jumper off the Pi4 USB-C power port.
It seems pointless, also I don't think there are any Test Pads, you'd have to create a USB -C power board with somewhere to jumper out, this isn't a PiZero.
(The large USB2 & 3 have pads, big solder points))
